The Supportive Oncology Collaborative developed a one-page follow-up reference document for each item/concern on the supportive oncology screening tool.
Each follow-up reference document has:
- Process/flow of the next steps to follow if a patient endorses an item
- Notes to address unique needs of patient populations
- Web links/handouts for patients to provide further information on the topic
- References and links to professional guidance that support the process/flow
While practitioners typically have an approach to address each item on the screening tool, their team may have differing approaches. A one-page summary for each item allows for standardization and consistency in approaching patient concerns. Further, the document lists content for additional information, which can also be shared with patients.
